Symphony of Evanston is seeking a Restorative/ Quality Assurance Nurse to join our dynamic team!

The Restorative/QA Nurse will:
  • Direct, supervise, and coordinator the Rehabilitation / Restorative Care Program
  • Make rounds on unit periodically ensuring resident safety devices are in place
  • Investigate all falls/incidents/accidents within 24 hours ensuring new interventions are put into place
  • Communication and collaboration with restorative tech to ensure that all safety interventions are on care cards, functioning and being used as ordered
  • Review charts of new admissions within 24 hours to ensure all orders are carried out and forms are completed
  • Implementation of a infection control program 
  • Assist with recruiting, interviewing and hiring team members for the Rehab/ Restorative program
  • Schedule PIC meetings, attend care conferences and monthly QAPI meetings
  • Ensure that assessments are completed according to MDS schedule
 

Candidate must have the following:
  • Great communication skills
  • Demonstrate a high standard of ethics
  • Dependability
  • Knowledge of nursing and medical procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities
  • Leadership and supervisory skills
  • Able to make independent decisions
  • Demonstrate excellent customer service
 

Requirements:
  • Licensed Practical Nurse (LPN)
  • Must be knowledgeable of the MDS process
  • Candidate must have restorative/ rehabilitation certification 
  • CPR Certified
